Mode of Selection : Written Test + Interview.

Fee & Mode of Payment : 
Payment should be made in  cash through any Canara Bank
Branch to Current Account No. 0403201003043 Canara Bank, Basavanagudi Branch, Bangalore.

Name of the candidate (in capital letter) and application no. should be mentioned in the pay in slip in the column “name of the remitter”. Fees can be paid in the bank close of banking hours  on 18.1.2013. 

However applications will close at 5.00PM on 18.1.2013. (Eighteenth January, Two thousand thirteen).

Posts in the Grade Pay (ranging from) Grade pay Up to Rs.2000/­ :  Application fee for General and OBC candidates is Rs. 125/­     

Posts in the Grade Pay (ranging from) Grade pay above Rs. 2000/­ upto Rs.4600/­ : Application fee for General and OBC candidates Rs. 225/­

Important Points to Note :
1.The vacancies notified are in various ESIC Medical institutions and Hospitals in Karnataka. Those candidates who are willing to accept the posting any where in the specified State and are willing to be transferred in any part of India on promotional posts, 
as per policy of ESIC, need apply.

2.There shall be negative marking for wrong answer. 
For each wrong answer 0.33 mark will be deducted.

3.Candidates are advised to apply for one post only as written examination for all the categories of posts
will be held on the same day and same time.

4. Only system generated application with two passport size photo copies and duly supported with
clean/ legible attested copies of the relevant certificates and   marks statements (in English or Hindi)
should be sent  by Registered post  / Speed post  before 25.1.2013

5.Original documents / certificates should not be sent with the application form

6.Envelope   containing   the   application   should   be   superscribed   “Application   for   the   post   of

Application No. _____________________ Category
           (General/SC/ST/OBC/PH/EX­Servicemen) ”. 

7.Admit Cards can be downloaded by using   candidates user name and password (User Name will be
candidates application no. and Password will be Date of Birth). 
Date of downloading the admit card will
be intimated in the same website.

Documents required to be attached with the application.
Attested copies of certificates in support of 

(i) Date of birth, 

(ii) Educational & Technical qualification along with marks statement, 

(iii) caste / community certificate (Candidates claiming reservation / age
relaxation on grounds of belonging to OBC should submit the 
Community Certificate in annexure “A”
prescribed vide Govt. of India,
Department of Personnel and Training OM No. 36012/22/93­Estt (SCT)
dated 08.09.1993 which  is modified vide G.O.I.,  DOPT’s OM No.  36033/3/2004­EST(Res.) dated
09.03.2004falling which the benefit of reservation / 
relaxation will not be given or their application
shall be rejected and no request / correspondence will be entertained). 

(iv) Working Experience, 

(v) Discharge certificate in respect of   Ex.   Servicemen,   

(vi)   Disability   certificate   in   respect   of   PH  candidates,  

(vii) Registration   certificate   (where   required).   

(viii)   Original counterfoil of  bank challan. 
Without these documents candidature will be rejected at the
