HOW TO APPLY :
(i) Application Form : Candidates are required to apply to the Special Secretary to the Commission in the prescribed short application form which, along with a brochure containing
general instructions for filling up the form, a copy of long advertisement containing the details of the recruitment, an acknowledgement card and a special envelope for sending the short application to the Commission’s office, can be obtained from the Commission’s office on payment of cash of Rs..30/- (Rupees thirty) only between 11.00 A.M. & 1.30 P.M. and 2.00 P.M. & 4.00 P.M. on any working day or by Speed Post/Registered Post on remittance of Rs..70/- (Rupees seventy) only by Bank Draft/Pay Order drawn in favour of Special Secretary, Odisha Public Service Commission payable at any Scheduled Bank at Cuttack with a forwarding letter addressed to the Special Secretary, O.P.S.C., Cuttack mentioning full name & address of the candidate, Advertisement Number and name of the Post/Service advertised in BOLD CAPITAL LETTERS.
(ii) If the letter along with Bank Draft/Pay Order from any candidate, for supply of prescribed application form, is received in the office of the O.P.S.C. after the last date
for receipt of applications, no application form will be supplied to such candidates;
(iii) Candidates must follow instructions meticulously for filling up the short application form as enumerated in the brochure enclosed;
(iv) APPLICATIONS WITHOUT REQUIRED AMOUNT OF FEE, WITHOUT PHOTOGRAPH, WITHOUT LEFT THUMB IMPRESSION AND DECLARATION NOT
SIGNED BY THE CANDIDATES (ALSO NOT SIGNED AT ALL THE PLACES
PROVIDED FOR SIGNATURE OF THE CANDIDATE) IN SHORT APPLICATION
FORM, APPLICATIONS WITHOUT CHOICE OF CENTRES, APPLICATIONS WITHOUT MENTION/WITHOUT DARKENING OF CODE NUMBERS OF THREE
OPTIONAL SUBJECTS AND/OR INCOMPLETE IN ANY RESPECT WILL BE
SUMMARILY REJECTED. SIMILARLY, APPLICATIONS RECEIVED LATE AND
APPLICATIONS FROM OVERAGED AND UNDERAGED CANDIDATES WILL ALSO
BE SUMMARILY REJECTED;
(v) Candidates are required to keep the Short (OMR) application form in the special envelope, supplied by the Commission’s office, after filling in the same.
The candidates must take due care in handling and folding the short application form.
(vi) Candidates are advised to send their applications in prescribed form by registered post /speed post/courier service to the Special Secretary, Odisha Public Service Commission, 19,
Dr. P.K. Parija Road, Cuttack-753001. The Commission will not take any responsibility if the
application is not received in time. They may also deliver their applications in closed covers
personally at the Commission’s office Counter against proper acknowledgement receipt on or before the last date for receipt of application on working days between 10.30 A.M. & 1.30 P.M. and 2.00 P.M & 5.00 P.M.
(vii) CANDIDATES SHOULD VERIFY THE FOLLOWING BEFORE MAILING/ SENDING THE APPLICATIONS:
(a) That you have used the Short (OMR) application form meant for ODISHA JUDICIAL SERVICE EXAMINATION purchased from the office of the O.P.S.C. only;
(b) That you have filled in all the relevant columns of the application form by blackening the appropriate circles by H.B. Pencil and also written the corresponding code(s) in the boxes. It may be carefully checked that there is no variation between the entries made by you by darkening the circles and those written in the accompanying boxes;
(c) That you have affixed your recent photograph in black and white (unsigned and unattested) in column-15 of the application form;
(d) That you have put your full signatures in the appropriate boxes against Columns 16 and 24 of the application form;
(e) That you have put your left thumb impression in the relevant box against Column No. 17 of the Short Application Form;
(f) That you have filled in the acknowledgement card (supplied along with the application form) and affixed postage stamp of Rs.6/- at the appropriate place of the card and inserted it into the special envelope meant for mailing your application to the Commission;
(g) That in case you are required to pay examination fee of Rs.100/-, the Original Treasury Chalan/Bank Draft/Pay Order, as the case may be, should be accompanied with your application and inserted into the special envelope before sealing the envelope by gum for despatch;
(h) That you have written the name of the Examination Zone, Examination Zone Code, your name and address (including PIN Code) in the appropriate spaces provided on the envelope meant for despatch of the application form;
(i) The special envelope should contain only three things, viz :
(i) Application Form duly filled in, signed and photograph affixed;
(ii) Original Treasury Chalan/Bank Draft/Pay Order; and
(iii) Acknowledgement Card duly filled in and affixed with postage stamp of Rs.6/- only. No other enclosure/documents/certificates should be attached thereof.
The aforesaid documents are not to be tagged/pinned/stitched to the short application form.
NOTE: (1) While filling in the short application form, the candidate should carefully
decide about his/her choice for the zone of examination. More than one application from a candidate, giving different zones of examination, will not be accepted in any case. Even if a candidate sends more than one completed application, the Commission will accept only one application at their discretion and the Commission’s decision in the matter shall be final.
If any candidate appears at a centre/optional subject (main written examination) other than the one indicated by the Commission in his/her admission certificate, the papers of such a candidate will not be valued and his/her candidature will be liable to cancellation.
(2) Since these short application forms are to be processed in a computerized system, due care should be taken by the candidates to fill up their short application forms correctly. Necessary instructions for filling up the short application may be seen in the Brochure supplied and relevant portion of the advertisement. No column of the short application form should be left blank. INCOMPLETE OR DEFECTIVE APPLICATIONS SHALL BE SUMMARILY REJECTED. No representation or correspondence regarding
such rejection shall be entertained under any circumstances.
(3) Candidates are not required to submit, along with their short application,
any copy of the certificate in support of their claims regarding age, educational qualification, Scheduled Castes/ Scheduled Tribes/ S.E.B.C./ Orthopaedically Handicapped or any other certificate. The relevant documents are to be submitted along with the long application for the main written examination, if selected in the preliminary written examination.
(4) The candidates, while applying for the preliminary written examination, should ensure that they fulfill all the eligibility conditions for admission to the examination. Their admission at all the stages of the examination; i.e. preliminary written examination, main written examination & interview will be PURELY PROVISIONAL, subject to their satisfying the prescribed eligibility conditions. If on verification at any time before or after the preliminary written examination, main written examination & interview, it is found that they do not fulfil any of the eligibility conditions, their candidature for the examination will be cancelled by the Commission. If any of their claims is found to be incorrect, they may render themselves liable to imposition of penalty as may be decided by the Commission in term of Rule-21 of the Odisha Superior Judicial Service & Odisha Judicial Service, Rules, 2007.
